ONLINE REGISTRATION

Please complete and submit the entire ONLINE registration form link below to speed your registration. (One form per participant. Please resubmit the form for additional registrants.) Incomplete forms cannot be processed. All forms must include payment, purchase order, or ATE Center codes (if applicable). Confirmations will be sent to the registrant. Invoices will be sent to the billing organization.

If you choose not to submit online, you may mail or fax your registration using the mailing or faxing information below.

CANCELLATIONS / SUBSTITUTIONS

For individuals who must cancel their registrations, the following rules apply:

  • All cancellations must be in writing.
  • A registrant whose cancellation is received by July 13, 2012, will be charged 50% of the total registration fee.
  • Individuals whose cancellations are received after July 13 and conference no-shows will be responsible for the full registration fee.
  • Those who determine they cannot attend the conference after they have registered may substitute colleagues in their place. Please email llocke@cord.org for further details on submitting a substitution request.
